Service Dog Team Identification Card

To be eligible to apply for Service Dog Team Identification Card, you must ordinarily live in Alberta and your service dog must have successfully completed a training program from a school or institution accredited by Assistance Dogs International, Inc..

To get an application form for a service dog team identification card, do one of these three things:

  1. Acrobat PDF Document Download and print the application form
  2. Request the form by emailing servicedogs@gov.ab.ca
  3. Call Alberta Seniors and Community Supports directly at 780-427-9136 and ask to speak to the Service Dogs Act administrator. To call toll-free, first dial 310-0000.

There are no application fees for a Service Dog Team Identification Card. Alberta Seniors and Community Supports manages the application process and distributes the identification cards. Alberta Seniors and Community Supports will review the application for completeness. They will contact the training school to confirm that your dog completed the accredited training, and to request a photo of you and your dog.

Upon receipt of these documents, your identification card will be processed. Your identification card and a copy of the Service Dogs Act will be mailed to you.
